Steps To Get Your Utha Medical Marijuana Card
1. Check Your Eligibility

2. Contact A Certified Physician
Once you get a vague idea that you’re suffering from one of the qualifying conditions mentioned above, it is now time for you to book an evaluation with a certified medical practitioner. The state requires you to submit a Medical Marijuana recommendation from a licensed doctor which will act as a decisive document for your application.

3. Complete Your Application And Recieve Your MMJ Card
Once you get the recommendation, the next step is to register yourself for a Utah ID account. Once you’re registered, you can now fill out your section of the application and leave the rest to your QMP. The QMP section includes the doctor’s medical recommendation and any other additional observation they want to mention. Once the application is complete, you can submit your application by paying the Medical Card fee which is as follows,

- Patient Card (first 90-day renewal): $5
